- Total amount in the patch is not the nominal amount delivered over 24 hours: the 9.5 mg/24h system contains 18 mg total rivastigmine base and has an area of 10 cm². Check mg/24h, total content, area, NDC and market together.
- The label identifies failure to remove an old patch and use of multiple patches as sources of serious medication errors. A caregiver record should separately confirm ‘old patch removed’ and ‘new patch applied’. If duplicate patches are found, do not calculate a remedy online; use local clinician, poison-control or urgent-care routes.
- Do not cut, fold, damage or alter a patch to obtain a ‘half amount’; a damaged pouch or patch is not the same as an intact product. Handling needs the current label for the exact product and pharmacist review.
