• The label warns that postural hypotension, dizziness or syncope can occur when starting treatment. Older adults, people with low blood pressure or falls, and those using antihypertensives should prioritise review of blood pressure, the complete medicine list and exact packs with a clinician or pharmacist; do not self-separate or stop a pack.
• Kidney function affects exposure. The historical label contraindicates severe renal impairment and specifies a different strength pathway for moderate impairment. The site does not select 4 mg, 8 mg, capsule count or timing from a laboratory value; the responsible team must assess the current product and prescription.
• Strong CYP3A4 inhibitors (including ketoconazole, itraconazole, clarithromycin and ritonavir in the label) are contraindicated, and strong P-gp inhibitors such as ciclosporin are not recommended.
