- Unusual sleepiness, inability to wake, slow or shallow breathing, blue lips, altered consciousness or suspected accidental ingestion/overdose are emergency cues requiring local emergency or poison services. Older or frail people, those with chronic lung disease and people also taking sedatives need priority review.
- Benzodiazepine sleeping/anxiety medicines, other sedative-hypnotics, alcohol, gabapentinoids, other opioids or anaesthetics may add to profound sedation, respiratory depression, coma and death. Separating clock times does not prove safety; review the complete medicine list.
- Some SSRIs, SNRIs, tricyclic antidepressants, triptans, MAO inhibitors, linezolid, lithium, St John's wort and other serotonergic medicines may increase serotonin-syndrome risk. A combination of agitation, fever, sweating, tremor, muscle rigidity, diarrhoea or mental-status change needs prompt medical assessment and emergency services if severe.
- Tramadol can increase seizure risk. A seizure history, head injury, alcohol/sedative withdrawal or another medicine that lowers the seizure threshold requires professional review. Do not self-add tablets or alternate another pain medicine because pain worsens.
